If a student is to receive any medication during the school day, the parent or guardian must bring the medication to the office and sign an Authorization for Administration of Medication form. Medicine must be properly labeled.
If a child requires medication during school hours and Firm Foundation Preparatory Academy does not have a signed authorization form on file, it will be necessary for a parent to administer the medication. In this case, a schedule must be set with the administration.
