Georgia law requires a certificate of immunization (Georgia Form 3231), and Ear, Eye and Dental form, (Georgia Form 3200) be updated and  file yearly in the school’s office for all students.  Immunizations records and Ear, Eye and Dental screenings can be updated yearly through care of through your physician or local health department authorities.  Accurate and completed forms must be submitted prior to the start of school.  New students transferring to Firm Foundation Preparatory Academy must have valid immunization records transferred from their previous school.
